Compounded semaglutide

How to get semaglutide: Wegovy direct, or a compounded vial

You can buy the approved injection yourself or go through a telehealth seller for a compounded vial. The injection at 0.25–2.4 mg has an equivalent calendar-month cost of $379.38 once its introductory rate ends; the second has a median of $199 across 410 US sellers, with a prescription required either way.

Quick answer

For compounded semaglutide, sign up with a telehealth seller, fill in its health questionnaire, and a prescriber licensed in your state decides whether to prescribe. Check when the seller charges you and what happens if the clinician declines. Approval and shipment timing vary by provider. The money question is the ongoing month, not the first one: 221 of 410 sellers advertise a figure lower than what their checkout bills over a calendar month, and 115 quote a price that holds only at the 0.25 mg starting dose.

Wegovy injection 0.25–2.4 mg: $349 per 28-day fill after any introductory offer. The comparison excludes oral Wegovy and HD 7.2 mg, which costs $399 per fill. Eligibility and other terms apply. All comparison amounts use 30.4375 days: each 28-day brand fill × 30.4375 ÷ 28. These are cost equivalents, not checkout charges. Manufacturer terms (checked October 2026).

Three routes to semaglutide2

  • Wegovy injection, bought direct. Listed on manufacturer terms at $379.38 per calendar month for injection 0.25–2.4 mg, after $199 for the first two fills at 0.25 mg and 0.5 mg. Compounded vs Wegovy works through that introductory rate.
  • Wegovy tablets. Semaglutide also comes as an approved pill: the same label covers a once-daily tablet with its own schedule, up to 25 mg. What the tablet is, and why a compounded “oral semaglutide” is not it.
  • A compounded vial through telehealth. 374 of the 379 sellers with a computed month charge less than $379.38. This is the route the rest of this page covers, and the index prices every one of them.

The telehealth sequence, and where money moves4

  1. Choose the seller (free). Sort by the computed month rather than the advertised one, and check the billing cycle: a price charged every four weeks lands thirteen times a year. The state pages narrow the list to sellers that ship to you.
  2. Answer the questionnaire (free at most sellers). It asks about your health history, medicines and the conditions the label excludes, including the thyroid cancer history behind the boxed warning. 29 sellers bill a recurring fee on top of the medication; they are on the fee list.
  3. Wait for the clinician (usually included). Review is mostly asynchronous and often done within two days. A seller with no clinician in the loop fails the legitimacy checks.
  4. Approval, then the charge. Introductory first months are common, so compare what month two and later will cost. How each seller’s headline compares with its checkout is on the transparency page.
  5. The vial ships cold (shipping normally included). Refrigerate it on arrival; the storage page says what to do if the ice pack has melted.

The 0.25 mg ladder and the bill1

Semaglutide climbs in uneven steps. The Wegovy label starts at 0.25 mg once weekly for four weeks, then moves to 0.5 mg, 1 mg and 1.7 mg, each step at least four weeks after the last, to a maintenance dose of 2.4 mg (or 1.7 mg). From 0.25 mg to 2.4 mg is almost a tenfold rise in drug per week. 115 of 410 sellers advertise a price that covers only the starting dose, so the month you compare at 0.25 mg is not the month you will pay at 2.4 mg. The lowest price that holds at every dose today is $75.59 a month. Every published price ladder is listed by dose, and the true-cost tool restates any quote as a calendar month.

Measuring the dose is the step FDA has warned about3

FDA has received adverse-event reports, some requiring hospitalization, that may be linked to dosing errors with compounded injectable semaglutide: patients drawing the wrong amount and, in some cases, clinicians miscalculating it. A 0.25 mg starting dose drawn from a multi-dose vial is a small volume, and the right mark depends on the concentration your pharmacy chose. Ask for the concentration in writing, follow the pharmacy label and your prescriber, and check the conversion with the units tool.

A low introductory charge can hide the ongoing bill. Our ElixaMD review separates its first-month offer from recurring refills and explains which services the price covers. Confirm those terms before enrolling.

Before prepaying several months, separate stopping renewal from getting a refund. Our Lean Meds review walks through its nonrefundable bundled terms, 72-hour cancellation notice and medical-disqualification exception. A lower monthly equivalent does not make unused months refundable.

Before you pay

  • Is the active ingredient semaglutide, not semaglutide sodium or acetate?
  • Which pharmacy fills the prescription, and is it licensed in your state?
  • Is the price per calendar month or per four weeks?
  • Is there a fee beside the medication?
  • What does the month cost at 1.7 mg and 2.4 mg, not only at 0.25 mg?
