Compounded semaglutide

Is compounded semaglutide still cheaper than Wegovy?

For most of the market, yes — and by less than the headline suggests, because the brand’s advertised price is not its ongoing one. At the expensive end of the compounded market the comparison has already inverted.

Quick answer

Wegovy sells direct at $349 a month once an introductory offer ends. Of the 397 compounded sellers with a price on this index, 389 come in under that and 8 do not. The median is $199.

What the approved product actually costs now1

The manufacturer’s direct page lists an original price of $1349.02 and a saving that brings it down. The ongoing out-of-pocket rate it states is $349 a month, at every strength on the selector — 0.25, 0.5, 1, 1.7, 2.4 and 7.2 mg.

That flatness is itself worth noting. The brand charges the same at the starting dose as at the maintenance dose, which a good deal of the compounded market does not — see price by dose.

The $199 is an introductory rate1

The number the page leads with is $199, and it is the number its structured data publishes to every price comparison engine that reads the page — an AggregateOffer whose low and high price are both $199.00.

The footnote underneath says what it actually is: the first two monthly fills, and only at the 0.25 mg and 0.5 mg strengths — the page's own footnote states $349/month after that. Two fills, two doses, and then a 75% increase. This site restates seller prices for exactly this reason, and it applies to the approved product as readily as to a compounding pharmacy — the difference is only that the brand prints the terms.

How that compares with the compounded market2

Against $349, the cheapest seller on this index is $53.81 and the median is $199. The saving at the median is real — roughly $150 a month — and it is a long way from the order-of-magnitude difference the compounded market was built on.

The distribution matters more than the median. This is not a market with one price; it is a market with a wide spread, and which side of the brand you land on depends entirely on which seller you pick.

Where the comparison inverts2

8 of 397 sellers on this index charge more per calendar month than the approved product does. For those, the compounded vial is not a cheaper alternative to Wegovy — it is a more expensive one, without the label.

278 sellers put a monthly price on their own page that their own billing does not produce. Nu Image Medical advertises $99.00 a month and bills $304.37 $205.37 more every month, or $2,464.44 over a year. Across all 278, the understatement comes to $16,992.12 a year. Every gap runs the same way: the advertised figure is always the smaller one. The chart shows the 8 largest; the rest are in the index.

Advertised is the seller's own per-month figure. True is that same charge divided by the days it covers, times 30.4375 — the average calendar month. A seller appears only where the two differ by more than 1%.

What you are actually trading3

Below the brand’s price you are buying a preparation with no label of its own, no FDA review of its manufacturing, and a concentration set by the pharmacy rather than by an approved specification. Above it, you are buying all of that and paying more than the approved product costs.

What is actually different →

And if the comparison you are really making is with the other compounded molecule — what the same sellers charge for tirzepatide — that is on compounded semaglutide vs compounded tirzepatide.
